Thursdays: 9 am - 5 pm ( 2nd and 4th Thursday of the month from 8am - 5 pm )
Saturday: 8 am - 12 pm
PHARMACY:
Monday - Wednesday & Friday: 8:30 am - 5:00 PM
Thursday: 9:00 AM - 5:00 PM
Closed daily from 12:30 PM to 1:00 PM

Our History
In 1994, Tepeyac began in a home on Kalamath Street. The community support which rallied around us more than 25 years ago is still what keeps us going today.

A Story of Our Work
Meet Maria, a 54-year-old monolingual Spanish-speaking woman, who arrived at Tepeyac with a health crisis. She had lost a tremendous amount of weight and had become frail. She couldn’t eat, couldn’t sleep, was so ill that she couldn’t work, and was incredibly frightened. Maria’s physical health problems contributed to mental health challenges as well.
“I was so sick that I thought that I might not make it home, that I wasn’t going to survive…The Tepeyac team gave my life back to me.”

Three days after Maria visited Tepeyac and was started on the right medication, she felt significantly better. And as a standard component of care at Tepeyac, Maria’s physical health provider conducted a behavioral health screening; after receiving behavioral services at Tepeyac, Maria no longer experiences elevated anxiety or depression.

Tepeyac is recognized by the National Committee for Quality Assurance as a Patient-Centered Medical Home. This means that we have made a demonstrated commitment to continuous quality improvement and a patient-centered approach to primary care.
Our model includes a focus on:
Comprehensive Care – We strive to meet a large majority of each patient’s physical and mental health care needs, including prevention and wellness, acute care, and chronic care.
Patient-Centered Approach – We strive to provide care that recognizes each patient as a whole person with unique needs, culture, values, preferences, priorities, and other factors in their lives that influence their health and treatment options. We aim to build strong relationships based in understanding and respect between patients and their care teams so that we can support patients and their families in learning how to manage and organize their own care at the level the patient chooses.
Care Coordination – We coordinate care across all elements of the broader health care system, including specialty care, hospitals, home health care, and community services and supports.
Accessible Services – We ensure that our services are accessible to patients with shorter waiting times for urgent needs, Saturday hours, and 24/7 telephone access to clinical advice.
Quality and Safety – We are committed to providing high quality, evidence-based care in collaboration with patients and their families. We measure our performance in many standardized metrics and patient experience and respond with continuous engagement in improvement activities.
As a patient-centered medical home, when you choose us as your primary care provider, we become your one-stop-shop for your primary care needs.
