Tepeyac Community Health Center provides team-based, affordable, quality medical, behavioral health, and dental care that fits the needs of your whole family. Most of the clinical staff are bicultural and bilingual, and all direct-service clinic providers are proficient or fluent in both Spanish and English. Since 1995, we have been proudly serving the community and we welcome you to choose Tepeyac as your medical home.
Tepeyac provides low-cost primary and preventive care at its clinic. All paid clinic providers and staff that deliver services directly to patients are completely bilingual. Medical Care
Tepeyac patients have access to a number of high-quality, evidence-based programs to support their health and wellness, including:

Preventative care, including: annual exams; well-child and male/female exams, including bone density screening; immunizations; physicals; mammograms through the St. Joseph Hospital Mammography Van, and STI (sexually transmitted infections) and pregnancy testing
Managed diabetes care, including: a comprehensive Diabetes Program that integrates services from across Clínica’s core program areas, including a health coach, discounted medication, diabetes management classes, exercise classes, healthy cooking classes and patient navigation
Personal health coaching, including: individualized health plans and ongoing support to promote patient activation (i.e., the skills, knowledge and confidence patients need to manage their health)
Referrals for specialty care at reduced costs
Screening and enrollment for Medicaid, including presumptive eligibility, and CHP+ enrollment
Tepeyac provides low-cost primary and preventive care at its clinic. All paid clinic providers and staff that deliver services directly to patients are completely bilingual, and all volunteer providers possess some level of Spanish fluency. In addition to general primary care services, patients have access to a number of high-quality, evidence-based programs to support their health and wellness, including: comprehensive prenatal care for expectant mothers; HIV/AIDS managed care that is overseen by one of Colorado’s leading infectious specialists; and a comprehensive diabetes program that includes personal health coaching, healthy eating instruction, and exercise.
With the expansion to our Vine St clinic, we are now able to offer more procedures, including:

Behavioral Health and Wellness
Tepeyac understands the unique needs of our patients. In addition to screening all patients for behavioral health needs, we provide counseling, resources, and case management for:
PHQ9 depression screenings are provided to medical patients, with referrals made for behavioral health services at Tepeyac, which include individual, couples and family counseling, as well as support groups (e.g, depression, chronic disease management).
Behavioral health offers both traditional, one-hour sessions and shorter interventions (known as brief intervention and/or warm hand-offs) for addressing immediate needs and crises. Both are offered at the clinic, improving convenience by removing barriers around transportation and scheduling appointments. Traditional counseling is also available at a second location less than two miles away (both in Denver).
Tepeyac integrates behavioral health care with physical health care by screening all patients for behavioral health needs. The Behavioral Health department has bilingual counselors who are available to respond when individuals and families are experiencing emotional distress. Providers are aware of the stressful challenges their patients face as well as the strengths of their patients’ culture (resilience, spiritual beliefs, family and community) that empower them to move past those challenges.

Health Promotions
At Tepeyac, we aim to be a beacon of wellness for entire communities. We provide coaching for chronic disease management and prevention, including diabetes prevention education, cancer awareness and prevention, healthy cooking, and exercise classes.
We offer cancer awareness, prevention and education, and referrals to mammograms through the St. Joseph Hospital Mammography Van.
Healthy cooking classes that promote healthy eating habits
Exercise classes (e.g., Zumba) at community locations
Health promotions, the third component of the integrated model, trains members of the local community to serve as peer-to-peer educators under the evidence-based Promotora de Salud model. Promotoras conduct bilingual, community-based workshops and provide one-on-one coaching on a number of topics aimed at empowering community members to effectively manage their health.

Telehealth Services
Telehealth, also known as telemedicine, is the ability to communicate face-to-face with your provider, even if you are in a different location! Patients can use telehealth to get medical care, behavioral health services, behavior change counseling, and health education. Telehealth is like Facetime or Skype with your provider, but with medical-grade HIPAA-compliant tools. Telehealth gives you and your family the most up-to-date medical care and education at a time and place most convenient for you.
During your telehealth visit, the provider will be able to see and hear you. You will see the provider on the video screen. Some virtual visits can also be completed through a phone call.

Sliding Scale Payment Plan
Tepeyac Community Health Center of Denver serves patients regardless of their ability to pay; no one will be denied health services due to financial barriers. We work to provide an income-based sliding fee schedule based on family size and household income, as well assistance with insurance coverage applications.
